Water/Sewer Forms
These forms are only to be filled out only by the property owner. Any other person completing these forms will cause them to become null and void. Identification must be presented to the Township in order to verify the account.
New Owner Form – If you just purchased a home in our Township. This form can only be completed by the New Owner. Any other party completing the form will make it null and void.
Seller Form – If you sold your home in our Township. This form can only be completed by the Seller. Any other party completing the form will make it null and void.
Landlord-Tenant Form – If you are a property owner and you have a tenant change. This form can only be completed by the owner of the property. Any other party completing the form will make it null and void.
Name Change Only – When a final read is not required, however, a name change needs to be made on the bill. This form can only be completed by the owner of the property. Any other party completing the form will make it null and void.
Address Change Only – To have the bill sent to a different address than on file. This form can only be completed by the owner of the property. Any other party completing the form will make it null and void.
Go Paperless E-Bill Form – By completing this enrollment form, you are choosing to receive your Thomas Township Water/Sewer Bill electronically. You will not receive a paper bill by mail.
Utility Bill Direct Debit Form – This form is for your utility bills only. Complete this form and return it with either a voided check, a recent statement, or a bank/credit union ID card showing your name, routing # and account #. The amount due on your water/sewer bill will be deducted from your bank account on the due date. To stop the direct debit, please complete a Stop Direct Debit Form.
Stop Utility Bill Direct Debit Form – This form is for your utility bills only. This form is to authorize the discontinuation of your direct debit account.

Tax Forms
Summer Tax Deferment Form – Deferring the summer tax bill means you have until the winter tax bill due date (last day of February) to pay your bill without penalty.
Tax Bill Direct Debit Form – This form is for your tax bills only. Complete this form and return it with either a voided check, a recent statement or a bank/credit union ID card showing your name, rounting #, and account #. The amount due on your tax bill will be deducted from your bank account on the selected withdrawal date.
Stop Tax Bill Direct Debit Form – This form is for your tax bill only. Complete this form if you would like to cancel your existing bank account information for your direct debit account.
